Job summary

AHN Healthcare@Home is seeking a Registered Nurse (RN) to provide skilled nursing care to patients in their homes in the Pleasant Hills, PA area. This role includes a $20,000 total bonus potential, with a $10,000 first-year success bonus and a $10,000 sign-on bonus paid within 18 months.

In this field-based home health position, you will travel to patient homes within your assigned territory, perform assessments, educate families, and ensure compliant EMR documentation.

Qualifications

• Active RN license in the state you work (PA).
• One year of RN experience in home health or related setting.
• Valid driver’s license and ability to travel to patient homes (approx. 20% of workday).

What You’ll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform patient assessments, medication management, and care planning
• Educate and empower patients and families on disease management and treatment plans so they feel confident, informed, and cared for
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory

RN Qualifications

• Active Registered Nurse, RN license in the state you work
• One year of RN experience and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
