Job summary

Residential Home Health and Hospice is seeking a Registered Nurse (RN) to provide skilled nursing care in patients' homes in the Halifax, PA area. Eligible for a $10,000 first year success bonus and ongoing rewards as you grow with the company.

You will deliver patient-centered care, perform assessments, educate families, and document care in EMR. A valid PA RN license, at least 1 year of RN experience, and a valid driver’s license with 20% travel are required. Equal opportunity employer.

• Perform patient assessments, medication management, and care planning

• Educate and empower patients and families on disease management and treatment plans so they feel confident, informed, and cared for

• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation

• Work independently, but never alone

• Travel to patient homes within assigned territory

RN Qualifications

• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
