Residential Home Health and Residential Hospice is seeking a Home Health RN to provide skilled nursing care in patients’ homes around Clarks Summit, PA. This role offers autonomy, meaningful patient relationships, and opportunities for growth in a nationally recognized home health and hospice provider.

You will perform patient assessments, medication management, and care planning with an interdisciplinary team, travel to patient homes within the assigned territory, and ensure thorough EMR

Qualificações

• Active RN license in state of work.
• Minimum 1 year RN experience.
• Current driver’s license and ability to spend ~20% of day driving.

What You’ll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform patient assessments, medication management, and care planning
• Educate and empower patients and families on disease management and treatment plans so they feel confident, informed, and cared for
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory

RN Qualifications

• Active Registered Nurse, RN license in the state you work
• One year of RN experience and the clinical expertise to deliver quality patient care
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
