Job summary

Doctor's Choice Home Care & Hospice Texas is seeking an experienced Registered Nurse to provide intermittent skilled nursing at patients' homes, following physician orders and a care plan. You will counsel patients and families, coordinate with physicians and other health care professionals, and help drive ongoing QAPI activities.

Supervisory duties include LPN and HHA oversight, with potential travel and on-call responsibilities.

Qualifications

• Licensed to practice as a Registered Nurse in the practicing states.
• Minimum 2 years of experience; home health preferred.
• CPR certification (American Heart Association or American Red Cross).
• Ability to exercise initiative and independent judgment.
• Strong oral and written communication; excellent interpersonal skills.

Responsibilities

• Administers skilled nursing care to patients on an intermittent basis per physician orders and plan of care.
• Participates in patient, caregiver, and family counseling and education.
• Communicates with physicians and other health care practitioners regarding the current plan of care.
• Contributes to the agency's QAPI program and participates in interdisciplinary assessments.
• Supervises care provided by Licensed Practical Nurse and Home Health Aide; participates in care plan development with patients and families.
