Job summary

Heart to Heart Hospice in Texas is seeking a Registered Nurse On Call to provide compassionate in-home nursing care. You will perform patient assessments, develop care plans, and coordinate with physicians and the IDG to deliver quality end-of-life care.

Ideal candidates are graduates of an approved Nursing program with an active RN license, a valid driver’s license, and reliable transportation. RN experience in healthcare settings is preferred; full-time benefits are provided.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.
• Current Driver's License, reliable transportation, and ongoing auto liability insurance.
• Two years of RN experience preferred in a healthcare/clinical setting.

Responsibilities

• Provide home visits for patient assessment, treatment, and support, including crisis management and admission.
• Develop and revise nursing care plans with patient, physicians, and IDG to meet goals.
• Utilize excellent customer service with internal and external customers; maintain professional phone etiquette.
• Prepare clinical and progress notes showing progress toward goals.
• Inform physician and staff of changes in patient needs and outcomes of care; monitor family response.
• Timely submission of all documentation.
• Ensure compliance with local, state, and federal laws, Medicare regulations, and policies.
