Job summary
Heart to Heart Hospice in Texas seeks a Registered Nurse On Call to deliver compassionate in-home nursing care to patients with life-limiting illnesses. You will perform patient assessments, develop care plans with physicians, document progress, and communicate changes to the care team.
A valid RN license, reliable transportation, and 2 years of RN experience are preferred. This full-time role offers benefits including medical/dental/vision, PTO, 401k matching, tuition reimbursement, and mileage
Qualifications
- Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
- Current State RN license.
- Driver's license and reliable transportation.
- Two years of RN experience preferred.
Responsibilities
- Provide home visits for assessment, treatment and support.
- Develop and revise nursing care plans with physician/IDG.
- Maintain professional phone etiquette when interfacing with clients.
- Document progress notes and outcomes.
- Communicate changes to physician and team; ensure timely documentation.
- Ensure compliance with laws and Medicare regulations and internal policies.
- Perform other duties as required.
