Job summary

Complete Home Care in Texas is seeking a registered nurse to administer in-home nursing care, coordinate with the interdisciplinary team, clients and families, and the referring agency.

Responsibilities include comprehensive assessments (including OASIS), planning and revising the Plan of Care, and ensuring quality, continuity, and compliant documentation while maintaining professional communication and on-call availability.

Qualifications

• One (1) year experience as an RN in a clinical care setting.
• Preferred, Home health experience.
• Nursing skills as defined as generally accepted standards of practice.
• Excellent interpersonal skills and ability to communicate effectively.
• Demonstrates proven decision making skills.
• If required to make visits, Hepatitis profile.
• Must read, write and comprehend English.

Responsibilities

• Promote /exemplify the Company's mission, vision, and values.
• Complete initial and ongoing assessments to identify needs of home health clients including OASIS.
• Regularly reevaluate client nursing needs and outcomes; assist in recovery or maintenance of life functions.
• Develop, initiate, and revise the Plan of Care to ensure quality and continuity of care.
• Provide effective communication to clients, families, and team members; participate in Case Conferences.
• Apply infection control concepts; coordinate and perform client care activities to protect clients and staff.
• Coordinate services and inform physician of changes in client condition.
• Monitor cases for third-party payer compliance.
• Complete timely documentation per company policy.
• Supervise and provide clinical direction to LVN/LPN, HHA, CNA.
• Perform on-call responsibilities and provide on-call services.
