Job summary

Heart to Heart Hospice seeks a Registered Nurse on call to provide home visits for patient assessment, treatment, and support, including crisis management and end-of-life care when needed.

The RN should have a nursing degree (Diploma/ASN/Bachelor), an active Texas RN license, and a valid driver’s license with reliable transportation and insurance. At least two years of RN experience are preferred. You'll plan care with physicians and document carefully.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.
• Current Driver's License with reliable transportation and active auto liability insurance.
• Two years of RN experience preferred in a healthcare/clinical setting.

Responsibilities

• Provide home visits for patient assessment, treatment, and support, including crisis management and pronouncement of death when needed.
• Develop and revise nursing care plans with patient physician and IDG; plan comprehensive service for patients and families.
• Exhibit excellent customer care with internal and external customers; maintain proper phone etiquette.
• Prepare clinical and progress notes showing progress toward goals.
• Inform physician and staff of changes in patient needs and intervention outcomes; evaluate responses.
• Submit all documentation timely.
• Perform other duties and ensure compliance with laws, Medicare regulations, and policies.
