Job summary

Heart to Heart Hospice in Texas seeks an on-call Registered Nurse to provide compassionate home visits, assessments, and support for patients with life-limiting illnesses. You will develop care plans in collaboration with physicians and the IDG, while maintaining strong communication with families.

The role requires an accredited nursing degree, an active state RN license, and a valid driver’s license with reliable transportation. Two years of RN experience is preferred to join our caring team.

Qualifications

• Graduate of an accredited Nursing Diploma/Associate/Baccalaureate program
• Current State RN license
• Current driver's license with reliable transportation and auto liability insurance
• Two years of RN experience in healthcare/clinical setting preferred

Responsibilities

• Provide home visits for patient assessment, treatment and support, including crisis management and pronouncement of death
• Develop and revise nursing care plans with patient physician and IDG
• Use excellent customer care skills with internal and external customers, including phone etiquette
• Prepare clinical/progress notes showing progress toward goals
• Inform physician and staff of changes in patient needs and outcomes
• Timely submission of all documentation
• Perform other duties as required
• Ensure compliance with local, state and federal laws and Medicare regulations
