Job summary

Doctor's Choice Home Care & Hospice Texas is seeking a Registered Nurse Case Manager to provide intermittent skilled nursing care in patients' homes, following physician orders and the plan of care under the supervision of the Director of Nurses.

You will educate patients and families, coordinate with physicians and other clinicians, supervise LVNs and HHAs, and contribute to ongoing QAPI assessments. This role involves 100% travel and requires strong communication and independent judgment.

Qualifications

• Currently licensed in the state(s) in which practicing
• Two (2) years experience; home health preferred
• CPR certified with American Heart Association or American Red Cross
• Ability to exercise initiative and independent judgment
• Strong communication skills, both oral and written
• Must have a current valid driver's license, auto liability insurance and reliable transportation
• Must be on call at least once per month
• 100% travel may be required

Responsibilities

• Administers skilled nursing care to patients in their place of residence under physician orders and plan of care
• Communicates with physicians involved in the plan of care and other health care practitioners
• Participates in patient, caregiver, and family counseling and education
• Supervises care of Licensed Vocational Nurse and Home Health Aide and contributes to QAPI program
