Granite Peak Home Health in Great Falls, MT is seeking a Registered Nurse to provide skilled home health care under the plan of care. You will perform initial visits, re-evaluations, and ongoing assessments while coordinating services and educating patients and families.

The role requires Montana RN licensure, BLS/CPR, and the ability to work independently with strong HIPAA knowledge. A valid driver's license and WellSky experience are preferred.

Formación

• Graduate of an approved school of professional nursing and currently licensed in Montana.
• Two years nursing experience, 1 year in Home Health preferred.
• Must hold a BLS/CPR certificate.
• Ability to exercise initiative and independent judgment.
• Familiarity with HIPAA compliance and patient privacy requirements.
• Knowledge of local healthcare market and community outreach strategies.
• Knowledge of software including Word, Excel, Outlook, WellSky (EHR).
• Clean driver’s license with a clean driving record and vehicle insurance (Preferred).

Responsabilidades

• Provides services by the plan of care.
• Makes the initial evaluation visit and reevaluates patient nursing needs.
• Initiates the plan of care and revisions.
• Provides services requiring specialized nursing skills.
• Initiates preventive and rehabilitative nursing procedures.
• Prepares clinical and progress notes for each patient visit and summaries of care conferences.
• Coordinates services and informs personnel of changes in condition.
• Counsels patient and family in meeting nursing needs.
• Participates in in-service programs and adheres to agency policies.
• Processes orders, notifies physicians, completes certifications and discharge summaries.
• Determines amount and type of nursing needed by each patient.
• Refers to therapists and social workers for specialized skills.
• Supervises and teaches other nursing personnel.
• Conducts patient care conferences and participates in QA/PI.
• Gives total patient care as needed and takes on-call duty nights/weekends/holidays.
• Completes and submits OASIS assessments and related documentation by policy.
