Granite Peak Home Health is seeking a qualified Registered Nurse to join our growing team in Montana. You will provide patient care per the plan of care, perform initial evaluations, and regularly reassess nursing needs in a home health setting.

Candidates should have an active Montana RN license and BLS/CPR certification. The role emphasizes independent nursing judgment, HIPAA compliance, and coordination with physical therapists, social workers, and other care team members.

Qualifications

• Montana RN license and graduation from an approved nursing program.

Responsabilités

• Provides services per the plan of care.
• Makes the initial evaluation visit and reevaluates nursing needs.
• Initiates and revises the plan of care as required.
• Provides services requiring specialized nursing skills.
• Performs preventive and rehabilitative nursing procedures.
• Prepares patient visit notes and care summaries per policy.
• Coordinates services and informs staff of changes in condition.
• Counsels patient and family on nursing needs.
• Participates in in-service programs.
• Adheres to agency policies and procedures.
• Manages orders, physician communications, and discharge summaries.
