Job summary

Granite Peak Home Health is hiring a Registered Nurse to join our growing team in Montana. You will provide nursing care per the plan of care, perform initial evaluations, and continuously reevaluate patient needs, while coordinating with the care team to deliver high-quality home health services.

We value initiative, HIPAA compliance, and strong communication with patients and families. A Montana RN license and BLS/CPR are required, with driving and flexibility for on-call duties.

Qualifications

• Licensed RN in Montana
• BLS/CPR certification required
• Knowledge of HIPAA and patient privacy
• Familiarity with Microsoft Word, Excel and Outlook

Responsibilities

• Provides services by the plan of care.
• Makes the initial evaluation visit and reevaluates nursing needs.
• Initiates plan of care and revisions.
• Provides skilled nursing services as required.
• Maintains clinical notes and summaries per policy.
• Coordinates services and informs team of changes in patient condition.
• Counsels patient and family on nursing needs.
• Participates in in-service programs and adheres to agency policies.
• Goes on-call as assigned.
• Completes and submits OASIS assessments and discharge summaries.
