Job summary

Granite Peak Home Health in Montana is seeking a Registered Nurse to join our growing team. You will deliver patient-centered nursing care under a defined plan of care, conduct initial evaluations, and adjust the plan as needs change.

The ideal candidate has MT RN license, BLS/CPR, and at least two years of nursing experience with one year in home health. Strong HIPAA knowledge and WellSky EHR proficiency are preferred.

Qualifications

• Graduate of an approved school of professional nursing and licensed in Montana.
• Two years nursing experience; one year in Home Health preferred.
• BLS/CPR certification required.
• Ability to exercise initiative and independent judgment.
• Familiarity with HIPAA and patient privacy requirements.
• Knowledge of local healthcare market and community outreach strategies.
• Proficient with Word, Excel, Outlook, and WellSky EHR.

Responsibilities

• Provide nursing services per the plan of care.
• Make initial evaluation visits and reevaluate patient nursing needs.
• Initiate and revise the plan of care as needed.
• Provide services requiring specialized nursing skills.
• Initiate preventive and rehabilitative nursing procedures.
• Prepare clinical and progress notes and summaries of care conferences.
• Coordinate services and inform staff of patient changes.
• Counsel patient and family; participate in in-service programs.
• Adhere to agency policies and procedures; process orders and discharges.
• Determine nursing needs per patient and refer to therapists when needed.
• Supervise and teach other nursing personnel.
• Lead patient care conferences; participate in QA/PI.
• Provide total patient care as needed.
• Take on-call duty nights/weekends/holidays.
• Complete and submit OASIS assessments and related documents on time.
