Granite Peak Home Health in Billings, MT is seeking a registered nurse to deliver in‑home care under the plan of care, conduct initial evaluations, and continuously reassess patient nursing needs.

The role requires MT RN licensure, 2 years of nursing experience (1 year home health preferred), BLS/CPR, HIPAA compliance, and proficiency with WellSky EHR and MS Office. A valid driver’s license is preferred.

Qualifications

• Graduate of an approved nursing program and MT license.
• Two years nursing experience; 1 year home health preferred.
• BLS/CPR certificate required.
• Ability to exercise initiative and independent judgment.
• Familiarity with HIPAA and patient privacy requirements.
• Knowledge of local healthcare market and outreach strategies.
• Proficient with Word, Excel, Outlook and WellSky EHR.
• Clean driver’s license with a good driving record (preferred).

Responsabilités

• Provides services by the plan of care.
• Makes initial evaluation visit and reevaluates nursing needs.
• Initiates plan of care and revisions as needed.
• Performs nursing services requiring specialized skills.
• Prepares care notes and summaries per agency policy.
• Coordinates services and informs changes in patient condition.
• Counsels patient and family; participates in in-service programs.
• Takes on-call duty nights, weekends, and holidays as assigned.
• Completes and submits OASIS assessments and discharge summaries.
