Job summary

Granite Peak Home Health in Billings, MT seeks a Registered Nurse to provide comprehensive home health care under the plan of care. You will perform initial and ongoing evaluations, initiate and adapt the care plan, and coordinate with PT/SLP/OT and social work as needed.

Requirements include MT RN license, BLS/CPR, and preferably 1 year home health experience. A valid driver’s license and WellSky EHR familiarity are preferred. Competitive salary and flexible schedule offered.

Qualifications

• Licensed Montana RN; graduate from an approved nursing program.
• Minimum 2 years of nursing experience; 1 year in home health preferred.
• BLS/CPR certification is required.
• Familiar with HIPAA, patient privacy, and healthcare regulations.
• Valid driver’s license with a clean driving record (preferred).

Responsibilities

• Provide nursing care per the plan of care.
• Perform initial evaluation visit and ongoing patient reassessments.
• Initiate and revise the plan of care as needed.
• Deliver services requiring specialized nursing skills.
• Coordinate services and inform team of changes in patient condition.
• Complete and submit OASIS/records as required by policy.
