Job summary

Granite Peak Home Health in Montana seeks a qualified registered nurse to provide care per plan of care. You will perform initial evaluations and regularly reevaluate nursing needs for home health patients.

The role requires a Montana RN license, 2 years nursing experience (1 year home health preferred), BLS/CPR, and proficiency with WellSky and MS Office. You may be on-call and will document care plans and coordinate with physicians.

Qualifications

• Must be a graduate of an approved nursing program and Montana-licensed.
• Minimum 2 years nursing experience; 1 year in home health preferred.
• Current BLS/CPR certification.
• Ability to exercise initiative and independent judgment.
• Knowledge of HIPAA and patient privacy regulations.
• Familiarity with healthcare regulations and local outreach strategies.
• Proficient in Word, Excel, Outlook and Wellsky (EHR).
• Clean driver’s license preferred.

Responsibilities

• Provides nursing services per the plan of care and conducts initial evaluation visits.
• Regularly reevaluates patient nursing needs and revises plans.
• Initiates plan of care and necessary revisions.
• Provides services requiring specialized nursing skills.
• Prepares clinical notes and care summaries.
• Coordinates services and informs physicians of changes.
• Counsels patient and family on nursing needs.
• Participates in in-service programs and adheres to agency policies.
• Performs OASIS assessments and discharge summaries.
• Gives total patient care and may take on-call duties.
