Job summary

Granite Peak Home Health in Montana is seeking a Registered Nurse to provide plan-of-care based nursing services. You will perform initial evaluations, regularly reassess patient needs, and implement revisions to the plan of care. The role requires independent judgment and compliance with HIPAA.

Responsibilities include coordinating services, documenting care, counseling patients and families, and on-call duties as scheduled. A strong grasp of WellSky EHR and MS Office is essential.

Qualifications

• Graduate of an approved school of professional nursing and licensed in Montana.
• Two years nursing experience; 1 year home health preferred.
• Must hold a BLS/CPR certificate.
• Ability to exercise initiative and independent judgment.
• Familiarity with HIPAA and patient privacy requirements.
• Knowledge of Word/Excel/Outlook and WellSky (EHR).

Responsibilities

• Provides nursing care per the plan of care.
• Initial evaluation visit and regular re-evaluations.
• Initiates and revises the plan of care as needed.
• Provide services requiring specialized nursing skills.
• Perform preventive and rehabilitative nursing procedures.
• Prepare and submit patient visit notes and care summaries.
• Coordinate services and inform changes in patient condition.
• Counsel patient and family; participate in in-service programs.
• Adhere to agency policies and procedures.
• Process orders and communicate with physicians; complete certifications and discharge summaries.
• Determine nursing needs per patient.
• Refer to therapists as needed and supervise other nursing staff.
• Participate in peer review quality assurance.
