Northwell Career Site is seeking a registered nurse to perform age-specific care using the nursing process, collaborating with physicians and the health team to coordinate procedures and treatments. You will exercise leadership in coordinating patient care and delegating activities on the unit.

The role involves comprehensive patient assessment across systems, psychosocial evaluation, outcome measurement, discharge planning, and education for patients and families.

Qualifications

• Graduate from an accredited School of Nursing, required.
• Bachelor’s Degree in Nursing, preferred.
• Current License to practice as a Registered Professional Nurse in New York State is required.
• May be enrolled in an accredited BSN program with defined timelines to completion.
• Valid Driver's License in New York State may be required depending on area of assignment.

Responsabilités

• Conducts age-appropriate plan of care using nursing process (assessment, diagnosis, planning, implementation, evaluation).
• Collaborates with physicians and health team to coordinate procedures and treatments.
• Uses leadership and clinical judgment to coordinate care and delegate on the unit.
• Interviews patients, explains policies, reviews charts, answers questions courteously.
• Assesses GI, cardiac, respiratory, renal, and neuro health; evaluates mobility, senses, prostheses, pain and skin condition.
• Communicates and documents physiologic health status and care plan.
• Assesses psychosocial health status; identifies teaching needs and coordinates support services.
• Identifies and measures patient outcomes; develops goals and prioritizes care activities.
• Implements plan of care; ensures safety and revises plan as needed.
• Administers prescribed treatments and uses equipment safely and timely.
• Keeps accurate documentation of treatment and patient responses.
• Acts during emergencies with calm, effective action and family communication.
• Plans for discharge and coordinates referrals and post-hospital care.
• Performs quality improvement tasks and analyzes outcomes for performance improvement.
• Collaborates with care team to plan and carry out treatment regimens.
• Delegates assignments ensuring timely completion of care.
• Implements skin care and wound management per protocol.
• Provides patient and family education about condition, treatment and safety.
• Performs other related duties as required.
