Qualifications

• Graduate from an accredited School of Nursing.
• Associate’s Degree in Nursing required.
• Bachelor’s Degree in Nursing preferred.
• Current license to practice as a Registered Professional Nurse in NY State and/or Connecticut.
• BLS required; specialized certifications as needed.

Responsibilities

• Assess patient physiologic health status.
• Assess patient’s psychosocial health status.
• Identify patient outcome and develop individualized goals.
• Formulate and prioritize plan of care and coordinate resources.
• Implement the plan of care and monitor patient progress.
• Evaluate care provided and document outcomes.
• Collaborate with physicians and care team, directing other staff as needed.
• Perform Decubitus care and skin care.
• Administer medications safely and accurately.
• Participate in patient and family education and discharge planning.
• Perform related duties as required.
