Responsibilities

• Coordinate with physicians and health team members in procedures and treatments.
• Assess psychosocial health status and patient education needs.
• Identify patient outcomes and develop individualized goals.
• Implement and revise the plan of care; document care plans and actions.
• Administer medications safely and educate patients and families.
• Collaborate with care team and coordinate discharge planning.

Education

Bachelor's Degree in Nursing (BSN) preferred

New York State Registered Professional Nurse license

BCLS, ACLS preferred
