Job summary

You will assess physiologic and psychosocial health, plan and implement care, document progress, educate patients and families, and support discharge planning.

Responsibilities

• Assesses the patient's physiologic health status.
• Assesses patient's psychosocial health status.
• Identifies patient outcome; develops criteria and sets individualized goals.
• Formulates the plan of care and sets priorities to achieve outcomes; coordinates supplies and medications.
• Implements the plan of care; maintains safety and reassesses as needed.
• Evaluates care provided and documents outcomes; participates in quality improvement.
• Collaborates with care team; delegates to Patient Care Associate and others as appropriate.
• Performs Decubritus care and skin care; applies dressings and aseptic technique.
• Administers medications correctly and safely; educates patients on drug interactions.
• Participates in patient and family education; plans discharge and coordinates referrals.
• Performs related duties as required.
