Suncrest Home Health, part of the Optum family, is hiring a Registered Nurse to deliver and coordinate nursing care in patients' homes. You will perform assessments, coordinate with physicians and the care team, and ensure compliance with laws and policies in a home-care setting.

Requirements include an active RN license in the state, a valid driver’s license with reliable transportation, and 1+ year of clinical experience. CPR within 90 days may be required.

Qualifikationen

• Active RN license in the state of practice.
• Valid driver’s license and reliable transportation.
• Preferred: 1+ year of clinical experience.
• CPR certification within 90 days of hire.

Aufgaben

• Develops, updates patient care plans in collaboration with physicians and the interdisciplinary team.
• Provides and coordinates high-quality clinical care; supervises aides and LPN/LVNs in compliance with regulations.
• Documents all patient care per policy and payer requirements; communicates with physicians, patients, families, and staff.
• Participates in orientation and precepting of new staff; contributes to quality and safety initiatives.
• Demonstrates professionalism through punctuality, ongoing learning, and adherence to standards; participates in on-call/weekend rotations.
• Able to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition.
