UnitedHealth Group is seeking an RN Care Manager for Suncrest Home Health, part of the Optum family. This office-based role oversees care plans and field staff, with a $20,000 sign-on bonus and a salary-based pay model plus annual bonus.

Open to licensed RNs, the position requires coordination of a multidisciplinary team to ensure high-quality home health care. Responsibilities include managing referrals, clinician assignments, patient start of care, and coordinating benefits.

Qualifications

• Current, unrestricted RN licensure in state of practice.
• Valid driver's license with access to a reliable vehicle or public transit.
• CPR certification or ability to obtain within 90 days of hire.

Responsabilités

• Provide clinical services within the scope of nursing practice per the plan of care.
• Receive referrals, assign clinicians, evaluate orders, and start care visits.
• Coordinate patient home health benefits, medical necessity, and insurance approvals.
• Ensure patient needs are continually assessed and care is individualized per physician orders.
• Review assessments and plans of care daily and consult clinicians as needed.
• Function effectively in diverse home situations regardless of patient demographics.
