Job summary

SunCrest Home Health is seeking a Registered Nurse to provide and direct nursing care to patients in their homes, in compliance with laws and agency policies. You will coordinate total plan of care with other health care professionals and ensure continuity of patient care by communicating with physicians, patients, families, and community resources.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License with access to a dependable vehicle or public transportation.
• Preferred: CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Initiates and revises the plan of care with the physician and other health care professionals.
• Supervises care provided by home health aides and licensed practical/vocational nurses, provides instruction, and assigns tasks according to state and federal regulations.
• Provides required supervisory visits and coordinates total plan of care.
• Completes comprehensive assessments (OASIS) including medication reconciliation accurately.
• Documents patient visits per policy and payer requirements, and syncs timely per policy.
• Communicates information with physicians, patients, and family to ensure quality care.
