Job summary

Suncrest Hospice is seeking a Registered Nurse to provide patient-centered care in homes and facilities within the service area. You will be part of an interdisciplinary team, offering psych-social assessments and coordinating community resources.

Roles include Case Management, Admissions, or Resource coordination. You will travel across the service area with reliable transportation, maintaining documentation and complying with hospice regulations.

Qualifications

• Current RN licensure in good standing in the state(s) of practice.
• Ability to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Ability to work 1-on-1 with patients while supported by a care team.
• Capable of independent work using phone, email and meetings with healthcare professionals.

Responsibilities

• Provide care for a group of patients in their homes or facilities.
• Work as part of an interdisciplinary hospice team.
• Roles may include Case Management, Admissions, or Resource coordination.
