Job summary

Suncrest Home Health, part of the LHC Group and within the Optum family, seeks an RN to provide and coordinate nursing care in patients' homes. You will collaborate with physicians and care teams to ensure continuity of care and patient safety.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• CPR certification or ability to obtain within 90 days of hire (preferred).
• 1+ years of Home Health experience (preferred).
• Ability to work independently and strong written and verbal communication.

Responsibilities

• Initiates, develops, implements, and revises the plan of care with the physician and other professionals.
• Supervises home health aides and licensed nurses; provides instruction and assigns tasks in line with regulations.
• Completes comprehensive assessments (OASIS) and medication reconciliation accurately and timely.
