Job summary

Suncrest Home Health, part of the Optum family, seeks a Registered Nurse for Home Health to deliver comprehensive nursing care in patients' homes. Responsibilities include care planning with the team, nursing assessments (including OASIS) and coordinating with physicians, families, and community resources to ensure continuity of care.

The role emphasizes high-quality clinical care, staff orientation, compliance with regulations, and participation in on-call rotations.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver’s License, vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Develops, implements, and updates patient plans of care in collaboration with physicians and the interdisciplinary team.
• Provides and coordinates high-quality clinical care; supervises home health aides and LPNs/LVNs.
• Documents patient care per policy and payer requirements; communicates with physicians, patients, families, and staff.
