Job summary

Optimal Home Care is seeking a Registered Nurse to deliver high-quality patient care across our Colorado service area. You will perform SOC/RECERTs, supervise LPNs, and coordinate care with facilities to meet patient needs.

The RN will complete documentation on time, participate in care coordination, and help grow census alongside the marketing team. A Colorado license, 1+ year RN experience, and a valid driver’s license are required.

Qualifications

• A graduate of an accredited, approved school of professional nursing.
• Active Colorado nursing license in good standing.
• Must communicate in the language of the client and document client related activity as appropriate.
• Current driver’s license.
• At least one (1) year of experience as a Registered Nurse; Home Health experience is a plus.
• Familiarity with OASIS preferred.

Responsibilities

• Complete SOC/ROC/RECERTs and supervisory visits on all appropriate patients at assigned accounts/territory (approx. 10-12 per week).
• Complete all required documentation timely per agency and Medicare guidelines.
• Provide RN care within scope of practice and supervise LPNs in assigned facilities.
• Manage care coordination and oversight of assigned accounts/territory; attend resident reviews.
• Provide consistent communications with facility staff and collaborate with marketing to support census growth.
• Prepare fiscal reports on assigned cases and collaborate with senior staff as needed.
• Work hours vary; minimum 40 hours per week, typically Mon-Fri.
