Adaptive Home Health, LLC is seeking a Physical Therapist Assistant (PTA) for field-based, home health care across Texas. The PTA will work under a supervising PT to deliver skilled visits, implement treatment plans, and drive functional mobility at patients’ homes.

You will coordinate with the care team, educate patients and caregivers, and document progress in Homecare Homebase. Field travel and strong clinical judgment are essential for success.

Qualifikationen

• Active Texas PTA license (in good standing).
• Strong clinical judgment and ability to work independently in the field under PT supervision.
• Comfortable escalating changes in condition to PT/RN/MD and documenting rationale clearly.
• Excellent communication and thorough documentation.
• Reliable follow-through on treatment plans, visit cadence, and patient goals.

Aufgaben

• Deliver skilled PTA visits in the home under the supervising PT’s plan of care.
• Execute treatment plans focusing on gait/balance, transfers, strengthening, and home safety.
• Set and track objective progress within PTA scope; communicate updates to PT.
• Drive multidisciplinary coordination and keep the care team aligned.
• Coach patients and caregivers with practical home programs.
• Document promptly in Homecare Homebase (HCHB).
