Job summary

Adaptive Home Health is seeking a Physical Therapist Assistant (PTA) for field-based home visits across Texas. The PTA will deliver skilled visits under a supervising PT, focusing on gait, mobility, safety, and functional gains in the home.

We offer a productivity-based compensation model ($58.00 per point; $75,000–$100,000 per year), schedule flexibility, and tech tools like Homecare Homebase to reduce documentation burden and streamline visit coordination.

Qualifications

• Active Texas PTA license in good standing.
• Ability to work independently in the field under PT supervision.
• Comfort escalating condition changes to PT/RN/MD with rationale.
• Strong communication and thorough documentation skills.
• Willingness to travel daily across assigned territory.
• Capable of transporting basic therapy equipment.

Responsibilities

• Deliver PTA visits in the home under supervising PT's plan of care.
• Execute treatment plans focused on mobility, safety, and function.
• Set and track progress; communicate updates to supervising PT.
• Coordinate with care team to address changes in condition.
• Coach patients/caregivers with practical home programs.
• Document promptly in HCHB with accuracy and consistency.
