Job summary

Adaptive Home Health in Texas is hiring Physical Therapist Assistants (PTA) to deliver skilled visits in patients' homes under a supervising PT. You will focus on gait and balance, transfers, safety, and functional mobility while documenting care in the EMR.

We offer per-visit compensation of $65 and a schedule designed for work-life balance, with daily travel across the territory. Must hold an active Texas PTA license and be able to travel within assigned area.

Qualifications

Active Texas PTA license in good standing.
Strong clinical judgment and ability to work independently in the field.
Comfort escalating changes to PT/RN/MD and documenting rationale clearly.
Strong communication and documentation skills.
Reliable follow-through on treatment plans and patient goals.
Valid driver’s license and willingness to travel daily.
Ability to carry/transport therapy equipment as needed.
Core PTA scope: safety-first mobility, gait training, transfers, home safety assessment.

Responsibilities

Deliver skilled PTA visits in the home under the supervising PT’s plan of care.
Execute treatment plans with a focus on functional mobility and measurable progress: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.
Set/track objective progress within PTA scope and communicate updates/concerns to the supervising PT.
Drive multidisciplinary coordination: escalate changes in condition, align on orders, and keep the care team moving.
Coach patients and caregivers with practical home programs that actually get done.
Document promptly and thoroughly in Homecare Homebase (HCHB) — defensible, efficient, and consistent.
