Qualifications

• Current license to practice as a physical therapy assistant in the assigned state.
• Graduate from a CAPTE-accredited PTA program.
• One year PTA/home health experience; mentoring option if less than a year.
• Current CPR certification.

Responsibilities

• Provides physical therapy services under the supervision of a PT per plan of care.
• Observes and reports patient responses and changes to the care team.
• Maintains documentation of supervisory activities in the medical record.
• Monitors patient responses to interventions and participates in quality activities.
• Supports follow-up care coordination with the admitting agency.
