Job summary

Adaptive Home Health seeks a qualified Physical Therapist Assistant (PTA) to deliver home-based visits under a supervising PT. The role emphasizes functional mobility, gait/balance, and home safety, with documentation in HCHB and productive, patient-centered care.

New hires benefit from an experienced clinical team and scheduling designed to maintain work-life balance. Travel across designated Texas territories is required; license and strong communication are essential.

Qualifications

• Active Texas PTA license in good standing.
• Strong clinical judgment and comfort working independently in the field under PT supervision.
• Comfortable escalating changes in condition and documenting rationale clearly.
• Strong communication and documentation, with reliable follow-through on plans and goals.
• Current driver's license and comfort with daily travel across assigned territory.
• Ability to carry/transport basic therapy equipment.
• Competency in core PTA scope: gait training, transfers, balance, home safety, and patient education.

Responsibilities

• Deliver skilled PTA visits in the home under the supervising PT’s plan of care.
• Execute treatment plans focusing on functional mobility and measurable progress.
• Set/track objective progress within PTA scope and communicate updates to PT.
• Drive multidisciplinary coordination and align orders for care continuity.
• Coach patients and caregivers with practical home programs.
• Document promptly in Homecare Homebase (HCHB).
