Job summary

Adaptive Home Health is seeking a Physical Therapist Assistant to perform skilled PTA visits in patients’ homes across Texas. You will work under a supervising PT to implement treatment plans that emphasize functional mobility, gait and balance, and home safety, with careful documentation in HCHB.

The role offers a full-time schedule Monday–Friday, field-based location, and a compensation model paid per productivity point. A Texas PTA license and strong clinical judgment are required.

Qualifications

• Active Texas PTA license in good standing.
• Ability to work independently in the field under PT supervision.
• Strong clinical judgment and home health competencies.
• Reliable communication and accurate documentation.
• Willingness to travel daily across assigned territory.

Responsibilities

• Deliver PTA visits in the home under the supervising PT’s plan of care.
• Execute treatment plans focusing on gait, transfers, strengthening, and home safety.
• Set and track progress within PTA scope and report updates to PT.
• Coordinate with care team to address condition changes and orders.
• Coach patients/caregivers with practical home programs.
• Document promptly in Homecare Homebase (HCHB) with defensible notes.
