Qualifications

• Active Texas PTA license (in good standing).
• Strong clinical judgment and comfort working independently in the field (PTA scope under PT supervision).
• Comfortable escalating condition changes to PT/RN/MD with clear rationale.
• Strong communication and documentation.
• Reliable follow-through on treatment plans, visit cadence, and patient goals.
• Current driver’s license and ability to travel daily across assigned territory.
• Able to carry/transport basic therapy equipment as needed.
• Competency in core home health PTA scope: safety-first mobility, transfers, gait/balance, home safety, and education.

Responsibilities

• Deliver skilled PTA visits in the home under the supervising PT’s plan of care.
• Execute treatment plans focusing on gait/balance, transfers, strengthening and functional mobility.
• Set/track objective progress within PTA scope and communicate updates to the supervising PT.
• Drive multidisciplinary coordination to elevate changes in condition and keep the care team moving.
• Coach patients and caregivers with practical home programs that get done.
• Document promptly and thoroughly in Homecare Homebase (HCHB).
