Job summary

Adaptive Home Health, LLC is hiring a Physical Therapist Assistant (PTA) for field-based home health in Texas. You will deliver skilled PTA visits under a supervising PT, focus on gait, transfers, strengthening, and home safety, and communicate progress and changes promptly.

The role supports a productivity-based pay model and requires active Texas PTA licensure with willingness to travel within the assigned territory.

Qualifications

• Active Texas PTA license in good standing.
• Strong clinical judgment and ability to work independently in the field under PT supervision.
• Excellent communication and thorough documentation.
• Willingness to travel daily across assigned territory.
• Carry/transport basic therapy equipment as needed.
• Competency in core PTA home health scope: safety-first mobility, transfers, gait, etc.

Responsibilities

• Deliver skilled PTA visits in the home under the supervising PT's plan of care.
• Execute treatment plans focused on functional mobility and home safety with measurable progress.
• Set/track objective progress within PTA scope and communicate updates to supervising PT.
• Drive multidisciplinary coordination with care team to adjust orders as condition evolves.
• Coach patients and caregivers with practical home programs to ensure adherence.
• Document promptly and thoroughly in the charting system.
