Job summary

BAYADA Home Health Care in Madison, New Jersey is seeking a full-time Home Health Physical Therapist to perform admissions Start of Care visits and coordinate patient care in the home setting.

The role requires an active New Jersey PT license, at least one year of PT experience, and OASIS experience with HCHB preferred; you will educate patients and families, document care, and work with a multidisciplinary team to promote safe, independent living at home.

Qualifications

• Active New Jersey Physical Therapist license
• Minimum of one year of recent work experience as a Physical Therapist
• OASIS experience required and HCHB strongly preferred

Responsibilities

• Make home visits for Start of Care admissions in designated territory
• Develop and implement individualized PT care plans including therapeutic exercises and gait training
• Document observations and evaluations in the medical record
• Educate clients and family members on rehabilitative care and activities for health and independent living
• Collaborate with nursing and other disciplines to adjust the care plan as needed
