Job summary

BAYADA Home Health Care, Inc. in Hoboken, NJ is hiring a full-time Admissions Physical Therapist to perform Start of Care visits and complete OASIS for adult and geriatric patients. The role requires NJ PT license, home health experience, and strong OASIS proficiency for accurate care planning.

Qualifications

• Current New Jersey Physical Therapist License.
• Minimum one year of recent work experience as a Physical Therapist.
• OASIS experience required; HCHB experience strongly preferred.

Responsibilities

• Make home visits to clients in designated geographic territories and conduct Start of Care (Admissions) visits.
• Perform diagnostic tests and measurements for mobility, gait, strength, balance and safety.
• Develop and implement individualized PT care plans with manual therapy, gait and balance training.
• Continuously assess and revise the PT care plan and coordinate with nursing in the multidisciplinary team.
• Educate clients and families on rehabilitative care and independence.
• Document observations and interventions using a touch pad tablet.
