• Make home visits for Start of Care admissions in designated territories.
• Perform diagnostics and measure mobility, balance, strength, and safety.
• Develop and implement individualized care plans with therapeutic exercises.
• Continuously assess and revise the PT care plan with nursing in the multidisciplinary team.
• Educate clients and family members on rehabilitative care and safety.
• Document observations and interventions accurately using a touch pad tablet.
