BAYADA Home Health Care, Inc. in Metuchen, New Jersey, is seeking a Home Health Physical Therapist for full-time admissions coverage. You will complete OASIS Starts of Care for adult and geriatric patients in Middlesex County, with a sign-on bonus and competitive base pay per patient visit.
One year of PT experience and NJ license required; home health and OASIS experience preferred. You will work within a multidisciplinary team to provide rehabilitative care at patients' homes, ensuring quality
Qualifications
- Active New Jersey Physical Therapist license.
- Minimum one year of recent clinical experience as a Physical Therapist.
- Home Health and OASIS experience strongly preferred.
Responsabilités
- Make home visits to clients in designated geographic territories.
- Perform diagnostic tests and measurements (mobility, ROM, strength, balance, safety).
- Develop and implement individualized care plans (manual therapy, gait training, balance).
- Continuously assess and revise PT care plan with multidisciplinary team.
- Educate clients and families in rehabilitative care for health and independence.
- Accurately document observations and interventions using a touchpad tablet.
