Qualifications

• A current New Jersey Physical Therapist License.
• A minimum of one year of recent work experience as a Physical Therapist
• OASIS experience required and HCHB strongly preferred

Responsibilities

• Make home visits to clients in designated geographic territories and conduct Start of Care (Admissions) visits.
• Perform diagnostic tests and measurements, such as mobility, transfer status, balance, and gait.
• Develop and implement individualized care plans, including manual therapy and gait training.
• Educate clients and family members in rehabilitative care and activities for health and independence.
• Accurately document observations and interventions using a touch pad tablet.
