Job summary

BAYADA Home Health Care in West Orange, New Jersey is seeking a full-time Admissions Physical Therapist to provide 1:1 home health visits, perform Start of Care assessments, and coordinate with a multidisciplinary team to deliver patient-centered rehabilitation.

Qualifications

• Active New Jersey Physical Therapist license.
• Minimum of one year of PT experience.
• OASIS experience required; HCHB experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories and conduct Start of Care visits.
• Perform mobility and functional assessments and safety evaluations.
• Develop and implement individualized care plans with PT interventions.
• Revise care plans as needed and collaborate with nursing on the multidisciplinary team.
• Educate clients and families on rehabilitative care and daily living activities.
• Document observations and interventions using the prescribed electronic system.
