Job summary

BAYADA Home Health Care, Inc. is seeking a dedicated Home Health Physical Therapist for a full-time role in Florham Park, New Jersey. You will perform Start of Care visits and create patient-specific treatment plans to help adults and geriatrics stay safely at home.

The role requires New Jersey PT licensure, at least one year of PT experience, and OASIS familiarity. A $10,000 sign-on bonus is offered for full-time admissions PTs.

Qualifications

• Current New Jersey Physical Therapist License.
• Minimum of one year of recent work experience as a Physical Therapist.
• OASIS experience required.

Responsibilities

• Make home visits to clients in designated geographic territories and conduct Start of Care (Admissions) visits.
• Perform mobility, range of motion, and functional assessments.
• Develop and implement individualized care plans including exercises and gait training.
• Revise PT care plans in coordination with nursing and multidisciplinary team.
• Educate clients and families on rehabilitative care for safe independent living.
• Document observations and interventions accurately in the electronic record.
