Job summary

BAYADA Home Health Care is seeking a Home Health Physical Therapist for a full-time role in Newark, New Jersey. The position involves conducting Start of Care visits, performing PT assessments, and coordinating with a multidisciplinary team to deliver skilled home health care.

Qualified candidates will hold a New Jersey Physical Therapist license, have at least one year of PT experience, and possess OASIS experience with HCHB preferred.

Qualifications

• A current New Jersey Physical Therapist License.
• A minimum of one year of recent work experience as a Physical Therapist.
• OASIS experience required and HCHB strongly preferred

Responsibilities

• Make home visits to clients in designated geographic territories and conduct Start of Care (Admissions) visits.
• Perform diagnostic tests and measurements, such as mobility, range of joints, transfer status, stability, ambulation patterns and appearance, strength and endurance.
• Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
• Continually assess and revise the Physical Therapy care plan and participate with nursing in the multidisciplinary care plan.
• Educate and instruct clients, family members, or other client representatives in rehabilitative care and activities necessary to promote health, safety, and independent living.
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided using a touch pad tablet
