Qualifications

• Active Texas Physical Therapist license.
• Professional liability insurance coverage for TX license.
• Graduate degree or higher in Physical Therapy (DPT preferred).
• Experience in home health or rehabilitative settings preferred.
• Ability to adapt care approaches to diverse patient needs.
• Up-to-date knowledge of evidence-based treatment techniques.
• Excellent communication and professional demeanor in patient education.

Responsibilities

• Evaluate patients’ physical abilities and functional limitations in a home health setting.
• Develop and implement individualized treatment plans to restore function and mobility.
• Provide interventions including exercise, gait training, manual therapy, and balance retraining.
• Educate patients and caregivers on home exercise programs and safety techniques.
• Monitor patient progress and adjust plans to achieve optimal outcomes.
• Document evaluations, treatment sessions, and progress notes using digital platforms.
• Collaborate with physicians, nurses, and occupational therapists.
• Conduct home safety assessments and recommend fall-prevention strategies.
• Deliver compassionate care and empower patients to maintain independence.
