Job summary

PARS Therapy is seeking a compassionate Physical Therapist to join our home health team in Liberty, TX. You will evaluate patients, design individualized plans, and provide interventions to improve mobility and reduce pain, all in patients' homes.

We value evidence-based care, strong communication, and the ability to adapt to varied patient needs. Active TX license and liability coverage are required, with a preferred background in home health or rehab settings.

Qualifications

• Active Physical Therapist license in Texas.
• Professional liability insurance coverage.
• Graduate degree in Physical Therapy from an accredited program.
• Experience in home health or rehabilitative settings preferred.
• Ability to adapt care approaches to patient needs.
• Up-to-date knowledge of evidence-based treatment techniques.

Responsibilities

• Evaluate patients’ physical abilities and functional limitations in a home health setting.
• Develop and implement individualized treatment plans to restore function and mobility.
• Provide therapeutic interventions including exercise, gait training, manual therapy, and balance retraining.
• Educate patients and caregivers on home exercise programs and safety techniques.
• Monitor patient progress and adjust plans to achieve optimal outcomes.
• Document evaluations, treatment sessions, and progress notes using digital platforms.
• Collaborate with physicians, nurses, and occupational therapists.
• Conduct home safety assessments to prevent falls or re-injury.
• Deliver compassionate care to empower patients toward independence.
