Qualifications

• Active Physical Therapist Assistant license in Texas.
• Current CPR certification.
• Experience in home health or rehab settings preferred.
• Ability to collaborate effectively within a multidisciplinary care team.

Responsibilities

• Deliver physical therapy services in the home under supervision of a licensed PT and per plan of care.
• Identify patient goals and implement interventions to meet mobility needs.
• Provide evidence-based treatments to improve movement and independence.
• Develop individualized care plans and educate patients for optimal recovery.
• Document services accurately and maintain patient records.
