Job summary

Qualifications

• Current license to practice physical therapy in the assigned state.
• Graduate from a CAPTE-accredited PT program.
• 1+ year home health PT experience or approval by regional clinical leaders if less than 1 year.
• Current CPR certification.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function.
• Provide interventions, conduct reexaminations every 30 days, modify interventions as necessary, and perform discharge planning.
• Other duties as assigned.
