Job summary

BAYADA Home Health Care, Inc. is actively seeking a Home Health Physical Therapist for ongoing, full-time visits in the Glen Arm/Baltimore, MD area.

You will provide skilled physical therapy services to adult and geriatric clients in home settings, collaborating with a multidisciplinary team to maintain safe, independent living at home. The role requires a current MD PT license and graduation from an accredited PT program.

Qualifications

• A current MD Physical Therapist - PT license.
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements related to mobility, strength, balance, and safety.
• Develop and implement individualized care plans including manual therapy, gait training, and balance interventions.
• Continually assess and revise the PT care plan in collaboration with nursing and the multidisciplinary team.
• Educate clients and families on rehabilitative care to promote health, safety, and independence.
• Accurately document observations and interventions using EMR and tablet.
